Asst. Year : 2011-12 (financial year 01-04-2010 to 31-03-2011)

  1. Name of the proprietor
  2. AN No.
  3. Name of the Business organization,
  4. Nature of Business
  5. Date of establishment
  6. VAT / TOT Registration
  7. Returns filed, Taxes paid with VAT Departments
  8. Loan from Banks, Financial Institutions, Limit available, Nature of Loan facility, Interest paid, Security offered.
  9. Vehicles, Movable Assets used for the business, ownership, Nos,. period of usage
  10. Current Account statements from Banks
  11. Loan Account statements
  12. TDS, TAN no. of the deductee
  13. Advance taxes paid. Self asst taxes paid.
  14. Tax paid details along with challan No. Bank, Branch with BSR and MICR code
  15.  LIC premiums paid: Name of the Company, Policy No. Date of payment, along with sum assured.
  16. Provident Fund (direct payment)
  17. Children School fee (tution fee only)
  18. Housing Loan repayment ( principal and Interest separately)
  19. Residence in own house/ rented house property
  20. Other savings with Nos. and Date of payment
  21. Infrastructure Bonds – Date of Payment
  22. Health insurance – evidence
  23. Dependent parents medical expenses along with evidence of expenditure
  24. Donations with evidence, Name and PAN No. of Institution, exemption certificate validity
  25. National Savings Certificates, Interest accrued thereon.
